CALL FOR APPLICATIONS FOR GETTING SOCIAL SUPPORT

CALL FOR APPLICATIONS: The social Support program for culturally and linguistically diversified citizens of City of Warsaw aimed at minimizing and preventing negative impact of COVID-19 pandemic.

Foundation for Somali invites to apply all the interested and needed foreigners residing in the City of Warsaw for a social support in the form of one food package that is provided to one household regardless number of people in this household.

The Applicant should meet the following criteria:

• be a foreigner (person staying on the territory of Poland but not holding Polish citizenship),
• hold a document issued or other type of documents confirming her/his legal stay in Poland,
• live in the City of Warsaw (one of 18 districts),
• suffer from negative impact of COVID-19 pandemic on life and livelihood conditions;
• suffer from difficult life situation beside economic situation, for example: health issues in family, caring for an elderly person, single household suffering from additional daily life challenges, single parent, unemployed, existing mental health problems in the family or people with disabilities, numerous (big) households.

An applicant should describe the level of his/her/their level of matching the criteria described above.

The social support in the form of one food package is provided to one household regardless number of people in this household.

Program documentation includes:

1. Application form for getting support by the program,
2. Program Rules and Regulations – electronic confirmation of reading and accepting the R&R,
3. Consent for sharing personal information and personal data – electronic confirmation of reading and accepting,
4. Copy of the document of the Applicant – attached to the electronic Request for support,
5. (optional) Additional documents  – attached to the electronic Request for support.

The application form should be filled in online and meet the criteria described. Deadline for sending your applications online is on Wednesday 16 December 2020 by 20:00.
